FCDS 2022 Consolidated Follow Back Audit (Archived)

The FCDS conducts annual follow-back audits and linkages with hospital discharge data to identify missed cancer cases. These activities ensure completeness of cases for certification and reporting.

FCDS matched the 2022 In-Patient and Out-Patient Discharges reported the facilities’ Finance-Billing/ Medical Records Department to the Agency for Health Care Administration (AHCA). All records with principal or secondary cancer diagnoses were linked to the FCDS database. A match was also completed of the Florida Vital Statistics Death Certificates for 2022. All non-matching records have been placed in your queue for review. The Consolidated Follow Back audit combines AHCA, Ambulatory Surgery Center (AMBI), and the Death Clearance follow-back process into a single follow-back queue. 

The 2022 Consolidated Follow-Back records are available in IDEA for review. 

The facilities are responsible for reviewing each case online. If the case is found to be not reportable, assign the appropriate disposition code; if the record was previously reported to FCDS assign disposition code 07, accession number, and sequence number, then press the Submit button. Any case found to meet the FCDS Cancer Case Reporting Requirements outlined in Section I of the FCDS DAM and found not to have been previously reported must be reported to FCDS. These are considered missed reportable cases. Assign a disposition code of 01, accession number, and sequence number to the reportable cases and press the Submit button.

These disposition code 01 records (missed cases) must be electronically reported to FCDS within 30 days of assigning the disposition code. After 30 days, the record(s) will be returned to the queue and marked as incomplete. 

The deadline to complete the review and submission of missed cases is September 1, 2024. 

Please remember that all FCDS audits are dictated and closely monitored by the Florida Department of Health. Facilities failing to meet the reporting requirements will be reported to DOH for non-compliance. Should you have any questions, please contact your Field Coordinator at (305) 243-4600.
