New Dermatology Physician Reporting Quick-Sheet

The Dermatology Reporting Quick-Sheet was developed to help Private Practice Physician users (Dermatology) download the IDEA application, create a PHYS IDEA user account, select the preferred reporting method, report, and answer frequently asked questions.

The Quick-Sheet has live links to facilitate user navigation and setup. All providers with a valid Florida License have an FCDS PPID linked to their FL License and are required to report their cases; this includes, but is not limited to, ME, PA, APRN, ARNP, DO, OS, etc. Cancer diagnosis and/or treatments should be reported by the provider using the preferred reporting method within 6 months of diagnosis and/or treatment.
